Performance steps	Preform an accurate medication card for a given fictive patient. Use a standard form or digital system for medication during the process. Ask patient/family to provide a list of medications and all supplements/alternative medications Compare medication list from patient medical record with the list provided by patient/family, with medication containers brought in with patient and new medication orders Delete any duplicate or discontinued medications. Include all current prescription and nonprescription medications from all sources. Confirm routes, dosages, and frequencies on reconciled list are correct. If possible review reconciled list with patient/family to verify accuracy Consult with prescribing clinician(s) to resolve discrepancies, as needed.
