Description

Preform an accurate medication card for a given fictive patient. Can be assessed during skills training as continuous feedback or by having a practical test. Steps highlighted are crucial for patient safety and must be preformed.

 

Use a standard form or digital system for medication during the process.

Ask patient/family to provide a list of medications and all supplements/alternative medications

Compare medication list from patient medical record with the list provided by patient/family, with medication containers brought in with patient and new medication orders

Delete any duplicate or discontinued medications. Include all current prescription and nonprescription medications from all sources.

Confirm routes, dosages, and frequencies on reconciled list are correct.

If possible review reconciled list with patient/family to verify accuracy

Consult with prescribing clinician(s) to resolve discrepancies, as needed.

Consult a licensed pharmacist or medication guide to identify potential medication interactions.

Assess patient/family understanding regarding purpose and possible adverse effects of all medications
