Psychosocial rehabilitation is defined in the Law on Social Services and
Social Assistance as a direction of social rehabilitation, which applies to a
person and his or her family and which aims to provide support in solving
psychosocial problems. Article 24 of the above-mentioned law stipulates that
... a social service or a rehabilitation service provider shall draw up an
individual social rehabilitation plan for ... a person. A psychosocial
rehabilitation plan is a description of services focused on the individual
needs of the client. Assessing psychosocial needs and identifying social
problems is the first step in developing a psychosocial plan. To determine
this, assessment skills and specific assessment tools are used. The second
step is to evaluate resources. The third step - planning of activities and
services. As psychosocial rehabilitation is a direction of social
rehabilitation, the basic components of the psychosocial rehabilitation plan
are specified in the regulations of the Cabinet of Ministers, which regulate
social rehabilitation. However, the components of the plan can be
supplemented, adapted and made even more detailed. The components of the
psychosocial rehabilitation plan within this skill are - defined service
name, client's psychosocial needs and problems, goals, tasks, responsible
employees, execution time, planned evaluation, recommendations, team
specialists who provide services.
